1. Registration & Back-to-School Checklist
- Complete SchoolMint Registration: All returning students must re-register, and new families must complete registration forms via SchoolMint. (Questions? Email [email protected] or call ext. 732).
- Submit Health Forms: Turn in all required health and immunization paperwork before day one. Fax number 413-743-7327
- *Please review the OTC Medication Protocol below and connect with the School Nurse ASAP if your student will take any prescription medications during the school day

- When to Stay Home: Keep students home for fevers (100.4°F+), active vomiting, or contagious illness. Read Health Guidelines

*Over-the-Counter (OTC) Medications
- An OTC medication consent must be completed before any over-the-counter medication can be administered at school.
- Verbal permission through phone calls or text messages will not be accepted.
- For the safety of all students, students are not permitted to carry or self-administer medication unless it is a provider-approved emergency medication, such as an EpiPen or inhaler.
- If a student is found with unauthorized medication, the medication will be removed, and a parent/guardian will be contacted.
- If you consent to OTC medication, you are consenting to the following medications and dosages:
Acetaminophen (Tylenol):
325mg tablets 2 tablets (unless otherwise indicated/directed)
Children's Tylenol 3 tablets (unless otherwise indicated/directed)
Headache, Fever, Dental Pain, Menstrual Cramps, Earache, New Sprains/Strains/Fractures, Significant Swelling or Bruising:
Ibuprofen 200mg tablets 2 tablets (unless otherwise indicated/directed)
Children's Ibuprofen 3 tablets (unless otherwise indicated/directed)
Stomachache, Nausea, Heartburn:
Tums 500mg chewable tablets 2 tablets (unless otherwise indicated/directed)
Significant allergic reaction with hives and/or swelling (not given for seasonal allergy symptoms):
Benadryl 25mg tablets
1-2 tablets (per directions)
Minor skin irritations (insect bite, allergic type rash):
Benadryl Cream
Minor coughing, sore throats:
Cough drops
Federal law requires secondary schools to release the names, addresses, and telephone numbers of students to military recruiters upon request. This applies specifically to students in grade 11.
