BART Timesheet
(For additional duties, overtime, etc. are due to the Business Office on the Friday before pay day and need to be signed by your supervisor.Please fill out the timesheet template and have your supervisor sign it.)
 
 
 
*Time off requests will be processed online now through the ADP portal. Please enter your time off as soon as you know about it and it will route to your supervisor for approval. For day-of call outs, please continue to use the call-out line as well.

Purchase Order Form
Please use this form when you want to make a purchase. Share it with your direct supervisor and email to [email protected] Please make sure it is editable.
 
Make sure to include:
-  your name & date
-  vendor name
-  vendor address including email address, phone number and contact/sales rep if known
-  quote number if known
-  total cost of items
 
After your supervisor approves it, the Business Office will approve then order unless you tell us otherwise. 
   
 In general, avoid using your own funds to make purchases. We need to ensure purchases are approved and budgeted and we can likely get better pricing when purchasing as a school.
 
BART Employee Travel Expense Form 
(Use this document to request reimbursement for travel expenses such as lodging, meals and mileage.)
 
BART Check Request for Reimbursement
(Use this document to request reimbursement for purchases/expenses that can not be made with a purchase order.)

Petty Cash Reimbursement
(Use this document to request reimbursement for non-travel purchases/expenses up to $25.)
 
(Use this to document damage to property that has financial implications (broken laptops, etc.). Save the template as a new document.)

Compliance

To be signed by each employee annually in December. Register on the site to be assigned to the training. Once completed, the certificate will be stored in the system, no need to turn anything in to the Business Office.

To be completed every two years by all employees. Register on the site to be assigned to the training. Once completed, the certificate will be stored in the system, no need to turn anything in to the Business Office.
